However, I think the brief answer is you always have to care about DDOS, the question is how few resources you need to dedicate to worrying about the DDOS.

For example, you could always write a custom validator for deciding what's a "good message" and what's a "bad message" and that will automatically drop the bad messages. However, you'll still have to decode and do a signature check before you can ignore the bad message. You could choose to track who sent you those bad messages and Blacklist the peer, and that will make the dropping of bad messages even more efficient.

I tested it in a public network and it works fine, however adding swarm.key breaks MDNS

Setting Swarm.EnableAutoNATService = true on bootstrap nodes and Swarm.EnableAutoRelay = true on clients allow the clients to find each other in local network (ipfs swarm peers shows mutable connections on 192.168.1.X)
